Blood Flow Restriction Therapy
Blood Flow Restriction (BFR) Therapy helps you build strength using a fraction of the weight you’d normally lift. Your Doctor of Physical Therapy (DPT) uses a medical-grade cuff to safely restrict blood flow while you exercise with low loads that are typically around 20% to 30% of your usual resistance.
Despite the lighter weight, BFR can stimulate muscle-building and strength responses similar to those achieved with heavier resistance training, while reducing the amount of stress placed on your joints.

BFR fills a gap that other rehab methods can’t. After knee replacement, ACL reconstruction, shoulder surgery, or injury, heavy loading may not be an option, and muscle loss can begin quickly. Your DPT helps you build strength with loads your joints can tolerate, helping reduce muscle loss, restore strength faster, and get you back to the activities you enjoy.
How Blood Flow Restriction Therapy Works
Your DPT applies a specialized cuff, similar in concept to a blood pressure cuff, to the upper part of your arm or leg. The cuff is inflated to a pressure set specifically for you, based on your individual limb occlusion pressure (LOP).
The goal is to partially restrict blood returning from the muscle to the heart while maintaining blood flow into the muscle. Using individualized pressure is important because BFR should not rely on a one-size-fits-all setting. Medical-grade equipment allows your DPT to precisely measure your LOP and control the amount of restriction.
With the cuff in place, you perform exercises using light resistance. The restricted blood flow creates changes within the working muscle, including a buildup of metabolic byproducts and increased cell swelling. These changes contribute to a muscle-building response similar to what occurs during heavy lifting. As a result, your muscles fatigue more quickly and are stimulated to adapt even though you’re using much less weight.
Research shows that low-load BFR can produce meaningful strength and muscle gains, making it especially useful when traditional heavy resistance training isn’t yet appropriate.
BFR isn’t a replacement for heavy lifting once you can tolerate it. It’s a way to keep building strength during the window when you can’t.
What to Expect During a BFR Session
BFR is part of your physical therapy session, not a standalone treatment. Your DPT incorporates it into your visit alongside exercises, manual therapy, and other treatments based on your individual plan of care.
A typical BFR exercise involves placing the cuff on your arm or leg and inflating it to your individualized pressure. You’ll then perform 3–5 sets of an exercise using light resistance. Depending on your needs, this might include quad extensions, leg presses, bicep curls, or even bodyweight exercises.
A common set structure is 30 repetitions followed by three sets of 15, with short rest periods between sets. The cuff is typically in place for 10–20 minutes during a session.
You’ll likely feel a strong muscle burn and fatigue—often much more than you’d expect from the amount of weight you’re using. That’s intentional. BFR keeps the load low while creating a high metabolic demand in the working muscle.
Who Can Benefit From BFR
Post-surgical rehabilitation: BFR can be especially useful after ACL reconstruction, knee replacement, rotator cuff repair and other joint surgeries when heavy loading is restricted early in recovery. Knee and hip arthritis: when arthritis makes heavy exercise painful or difficult to tolerate, BFR can help you build strength using loads that are easier on the joint. This makes it a useful option for patients who struggle to strengthen effectively with traditional resistance training.
Older adults and age-related muscle loss (sarcopenia): maintaining muscle strength becomes increasingly important with age, but joint pain, balance concerns, or other limitations can make heavy lifting difficult. BFR provides another way to stimulate strength and muscle growth using lighter resistance.
Athletes returning from injury: when an injury limits how much you can lift, BFR can help maintain or rebuild muscle and strength without requiring heavy loads. This can help bridge the gap between early rehabilitation and a return to full training.
Tendon pain and tendinopathy: Growing research supports BFR as a way to help patients with patellar tendinopathy, Achilles tendinopathy, and rotator cuff-related pain by allowing muscles to be trained with lighter loads. When traditional resistance exercise is painful or poorly tolerated, BFR can provide a more comfortable way to retrain joint muscles and tendons.
Who Performs BFR
Every BFR session at FX Physical Therapy is performed by a licensed Doctor of Physical Therapy (DPT) trained in blood flow restriction therapy. Safety
BFR is considered safe when performed by a trained clinician using medical-grade equipment and individualized pressure settings. As with any resistance exercise, you may experience muscle fatigue and burning during the session and some soreness afterwards.
The equipment and how BFR is applied matter. Medical-grade BFR systems measure your individual limb occlusion pressure (LOP) and allow your DPT to set the cuff to a precise, personalized pressure. Generic wraps or bands do not provide the same level of pressure control or monitoring and are not equivalent to clinical BFR.
Before your first session, your DPT will review your health history and screen for conditions that may make BFR inappropriate. If BFR isn’t appropriate or safe for you, your DPT will recommend an alternative approach.

What to Expect Afterwards
After BFR, it’s normal to feel fatigue or soreness in the muscles you trained. This is similar to what you might experience after a challenging workout, even though you used much lighter weights. You may also notice some temporary swelling in the treated limb after the cuff is removed. This typically settles quickly.
For most patients, BFR doesn’t require downtime, so you can return to your normal activities the same day.
Like traditional strength training, BFR takes time to produce results. Expect strength and muscle changes over several weeks, not after a single session. As you get stronger and your joint becomes ready for heavier loading, your DPT will gradually progress your exercises and adjust your BFR program.
